Obesity Management -
Healthy Lifestyles Program (HeLP)


The Healthy Lifestyles Program (HeLP) is designed to empower you and your clinic staff to diagnose, treat and offer prevention strategies to combat child obesity. Child obesity is present across all income levels, however almost half of poor children are overweight or obese. HeLP educators work directly with providers and their staff to enhance their methods for dealing with child obesity.

Ultimately, the goal is for your clinic to have a comprehensive program uniquely tailored to your office to diagnose and treat child obesity. We strive to provide materials and support to address child obesity with the families you serve.

We offer support for implementation of the Healthy Lifestyles Program through clinic shadow time where an educator can model motivational interviewing techniques for the staff or providers. Program completion activates a billing code for healthy lifestyle education with CMFHP members. Along with education credits, we provide a "tool box" filled with education materials for the clinic’s use as well as tools to measure BMI and BMI percentile. We encourage offices to use education materials for all patients, not just CMFHP members (applies to Primary Care Providers).

Healthy Lifestyles Program Health Coaches
As part of the comprehensive services that CMFHP offers to members and providers to address child obesity, we have health coaches that can work directly with members to support them in making healthy lifestyle changes. The health coaches offer different levels of service to meet the needs of the member. Levels of service include phone education, mailing packets of information and health coaching.

What is a health coach?
  • Someone who provides and reinforces weight management and healthy lifestyle education with families.
  • Someone who offers support to family members who are struggling with weight management and healthy lifestyle changes.
  • Someone who assists families in identifying goals, focusing on one behavior at a time for sustainable change and empowering families to make changes.
Refer a CMFHP member
Please use this form to refer your patients to our HeLP health coaches when needed. The health coaches assist in reinforcing the education provided by the clinic.

Forms

We would like to provide several resources to get you started if you are interested in enhancing your practice to better diagnose and treat child obesity. We can also serve as a resource to your practice through the HeLP program if you would like support in implementing a program in your clinic to diagnose and treat child obesity.
  • The Weight Management Algorithm summarizes recommendations for diagnosis and treatment of overweight and obesity. It also provides some commonly used codes for co-morbidities associated with overweight and obesity, including the diagnosis code for overweight and obesity (278.00). Since overweight and obesity is considered a chronic condition, diagnosing the problem is the first step in treatment.

  • The National institute for Children’s Healthcare Quality created an implementation guide for the diagnosis and treatment of child overweight and obesity. The guide provides action steps to treating child obesity.

  • The Health Habits Assessment (English, Spanish) reviews your patient’s current habits as they relate to obesity risk. We recommend using the assessment on well child visits for children 2 and older. It can help facilitate a discussion with your patients on areas they want to change. It also be a prevention tool for families with habits that may lead to obesity and other chronic conditions.
  • Adult Health Habits Assessment - reviews your patient's current habits as they relate to obesity risk. It can help facilitate a discussion with your patients on areas they want to change.
