Asthma Management Program
The Asthma Management Program creates a unique partnership between the health plan, the providers and the patient. It places asthma management where it is needed most - with the provider and the patient. Our certified asthma educators work with the staff and providers in each office to enhance and build a high quality, comprehensive asthma program that fits within the culture of each individual practice. After completion of this training session, Primary Care Providers (PCP) can be reimbursed for providing proper asthma education to their patients. If your primary care office is interested in being a part of this rewarding and beneficial opportunity, please contact us.Resources for participating offices include:
- a spirometer to use during the intervention
- asthma action plans
- educational resources and handouts
- reimbursement for asthma education (applies to Primary Care Providers)
- access to Asthma Health Coaches
- ongoing partnership and education
These forms and educational tools can be shared with all patients that have asthma. They are designed to reinforce asthma education provided by the PCP and empower the patients to self manage their chronic disease.
