Asthma Management Program


Living with asthma is a challenge! Our experienced and trained Asthma Health Coaches are available at no cost to you. Health coaches give you education and support as you learn about asthma symptoms, triggers and medications.

Our Asthma Management Program is a partnership between us, the provider, and you - our member. Health coaches assist you toward your goal of well-controlled asthma. We help you understand asthma and your Primary Care Provider's (PCP's) plan of care.

Health Coaches can meet with you in your home, at your PCP clinic, or in a public place (library, fast-food restaurant). We provide routine phone contact based on your needs. We work with you on meeting times that fit your busy life.

Health Coaches are a phone call or email away! Ask your PCP to complete a referral for you, or, call Customer Service toll-free at 1-877-347-9363 (KS) or 1-800-347-9363 (MO).
