Important Information for Members of a Federally-Recognized American Indian or Native Alaskan Tribe
Is your child a member of a federally-recognized American Indian or Native Alaska tribe? If so, you will not have to pay a premium for your child's health care coverage. Please contact the HealthWave Clearinghouse at 1-800-792-4884. You can also fax your notice to 1-800-498-1244. They will assist in updating your records.
CMFHP will communicate to Indian Health Service Clinics and CMFHP members who have identified themselves as American Indian/Alaskan Native that you are allowed to receive services from Indian Health Service entities without restrictions.

Tips on submitting a HealthWave application:
Send the required verification documents along with the application.
If the state needs more information, they will send you mail. Open all of your mail.
If you have to fax something to the state, write the Social Security Number, case number, or name and Date of Birth on each page you are faxing.
If you fax information to the state, know the fax number it was sent from (the Clearinghouse will ask you this when you call).
If you lost your job less than 6 months ago, send the state the following info:
- name of the employer
- last day of employment
- copy of the last pay stub.

Children’s Mercy Family Health Partners (CMFHP) is a not-for-profit safety net health plan. We offer low- or no-cost health insurance for eligible children and families through Kansas HealthWave.
You must apply for HealthWave to see if you or your family is eligible. The State of Kansas has eligibility guidelines for HealthWave. You must meet these guidelines to be eligible.
Eligibility ... Enrollment ... What's the difference?
Eligibility for HealthWave comes first. After you are eligible, then you can enroll in a health plan.
The state decides who is eligible, or qualified, for Kansas HealthWave.
Once the state says you are eligible for HealthWave, you can choose to enroll in a HealthWave plan. CMFHP is one of the plans you can choose.
To see if you or your children are eligible for HealthWave, you must apply. You can get an application by calling the HealthWave Clearinghouse at 1-800-792-4884. You can download and print an application from the Kansas HealthWave website. You can also get one at your local SRS office, school, or library.

Once you or your family become eligible for HealthWave, you will get an enrollment packet. The enrollment packet will help you choose a managed care health plan. CMFHP is one of the plans you can choose.
