Our Asthma Management Program includes a partnership between the health plan, the provider and you, our member. We assist you toward your goal of well-controlled asthma by helping you understand asthma and your Primary Care Provider’s (PCP's) plan of care.

Living with asthma is a challenge! Experienced and trained Asthma Health Coaches are available at no cost to you offering education and support as you learn about asthma symptoms, triggers and medications. We are here to help you develop your asthma self-management skills and to assist you in talking with your PCP about your Asthma Action Plan.

Health Coaches can meet with you in your home, at your PCP clinic, or in a public place (library, fast-food restaurant). They will provide routine phone contact based on your needs. They will work with you on meeting times that fit your busy life. Health Coaches are a phone call or email away! You may contact us to learn more about this benefit or ask your PCP to complete a referral for you.
