We hope that this quarterly publication will meet some of the needs you face as you reach out to the community. Children's Mercy Family Health Partners Role in MC+ Managed Care

Children's Mercy Family Health Partners (CMFHP) provides low-cost or no-cost health care insurance to the medically vulnerable population through a partnership with the State of Missouri's MC+ Managed Care program.  We serve individuals who live in the Western region of the state of Missouri

CMFHP focuses on ensuring all members receive health care services with effective and efficient coordination of care to promote clinically appropriate, timely, and high quality care.  Our staff of nurses and social workers are available to members for care coordination including: asthma management; improving access to primary care services; first touch (OB) case management; well women care; improved lead testing rates; and a nurse advice line.

CMFHP community relations representatives serve as liaisons between CMFHP and the community.  Their main role is to share resources with members and potential members by performing community outreach in designated territories.  They do this through presentations, in-service sessions, and personal visits.  They also coordinate participation in our sponsorship of health and safety related events.  Community Representatives are responsible for generating and/or following up on new outreach leads, maintaining a presence and involvement with select community organizations, and planning community events for Children’s Mercy Family Health Partners.

Change in Interpretation Provider

We have changed to Propio Language Services for interpreter services. If someone does not speak English, they can call 1-800-347-9363 to ask for help. We can help those who do not speak or understand English. We will get them a translator when needed. Hundreds of languages are offered including Spanish, Vietnamese, Somalian, and Low German. We want to help you with this service.

CMFHP Asthma Management Program

The fall asthma season is upon us and we have identified our high-risk asthmatic members who visited the ER and/or hospital last fall for their asthma. We have sent an educational mailing to the member, with a copy to their Primary Care Provider, reminding them to identify and avoid their asthma triggers and seek care through their Primary Care Provider before their asthma gets out of control this fall. We also encouraged them to discuss their asthma action plan with their Primary Care Provider to ensure it is updated with their current condition. If you have any questions about our mailing or would like to discuss one of our members with an Asthma Health Coach, please contact:

Coverage For All Tools

Members or potential members are often confused about the options for health insurance that are available to them. The Foundation for Health Coverage Education (FHCE) has a website. This is a great tool to help individuals see if they might be eligible for free or low cost health coverage. The FHCE offers four unique ways to help find health coverage. They are:
