CMFHP Asthma Management Program The fall asthma season is upon us and we have identified our high-risk asthmatic members who visited the ER and/or hospital last fall for their asthma. We have sent an educational mailing to the member, with a copy to their Primary Care Provider, reminding them to identify and avoid their asthma triggers and seek care through their Primary Care Provider before their asthma gets out of control this fall. We also encouraged them to discuss their asthma action plan with their Primary Care Provider to ensure it is updated with their current condition. If you have any questions about our mailing or would like to discuss one of our members with an Asthma Health Coach, please contact: Michelle Fulton, RRT, AE-C Pam Runyon, MSMFT, LBSW
