It's Still Up to You to Renew!

Most MO HealthNet health plan members must go through an annual review. Make sure to open all mail from your Family Support Division Office (FSD) and respond right away to all requests for information.

When it is time for your annual review, you may be mailed a letter from your local FSD office. The letter will tell you what you need to do to continue your MO HealthNet health coverage. Fill out the form completely and return it by the due date on the letter or you may lose your MO HealthNet coverage. With the form you will also need to provide one of the following:
  • current wage stub
  • current W-2 form
  • statement from your employer
Contact your local FSD office if you move or if your telephone number changes. The FSD will then have your correct information for mailing you the annual review notice.

It is up to you to renew! We want you to keep your MO HealthNet coverage so we can continue to serve you!



Premiums are Due Every Month!

Families of 46,000 Missouri children with MO HealthNet coverage will begin paying monthly premiums in September 2005. If this change happens for your child you will get a letter from your Family Support Division (FSD) case worker.

You must make each payment on time. If you don't make each payment your child may lose MO HealthNet coverage and may not be eligible for MO HealthNet again for six months!

There is a great way to make sure your payment gets in on time. You can have the payment taken out of your checking account each month. It is easy and safe! To request this service, call the Premium Collection Unit at 1-877-888-2811.

Make sure to pay your monthly premium so you can keep your child's MO HealthNet health coverage!

What Changes Do You Need To Report To Your Caseworker?

If you move, it is important that you report your new address by calling your local Family Support Division Office and the MO HealthNet Managed Care Enrollment Helpline at 1-800-348-6627. Then call Children's Mercy Family Health Partners at 816-855-1888 or 1-800-347-9363. Your MO HealthNet Managed Care coverage may be affected. If we do not know where you live, you will miss important information about your coverage. Changes you need to report to your local Family Support Division office include:
  • family size (including the birth of any babies);
  • income;
  • address;
  • phone number; and
  • availability of insurance.
