Go to Provider Search Page
To change your PCP you will use the provider search page. Once you have selected your search options and clicked on the search button, the results
will be displayed. The results page should look similar to this:
Providers that can be selected as a PCP will have a plus sign in the column marked Pick PCP. To request a provider as your PCP, click on the plus
sign and a form will popup that will look like this:
Fill out the form with the correct information and click on the "Request PCP" button to request the provider as your PCP.
