Children's Mercy Family Health Partners ("CMFHP") recognizes the importance of
protecting the privacy of our members and visitors to our website while permitting
us to conduct legitimate business by providing information of interest
to our members and visitors. Our privacy policy is stated below.
What this Privacy Policy Covers
This Privacy Policy covers our treatment of personally identifiable infor-
mation that we collect when you are on the CMFHP website and when you
use services provided by CMFHP. This policy also covers our treatment of
any personally identifiable information that may be provided to us from
other sources. This policy does not apply to the practices of companies
that we do not own or control or to people that we do not employ or
manage.
Information Collection
We collect personally identifying information from our users, members
and visitors. We recognize two kinds of personal data that deserve
different levels of protection:
-
Personally identifiable information includes, for example, your
name, postal addresses, e-mail addresses, billing information,
your title or degree, your organization, your professional
specialty, answers to survey questions and "click stream" data
that tracks visitor activity.
-
Sensitive data includes, for example, Social Security numbers,
passwords, mothers' maiden names, personal financial data,
information about specific medical conditions and credit card
information.
CMFHP collects personally identifiable information and sensitive data only
when there is a legitimate need to do so. If your relationship with us
includes providing us with sensitive data, we will endeavor to protect
that information. Except as provided herein, we will not distribute sensi-
tive data outside of CMFHP or our affiliates, and will give you the chance
to "opt out" of sharing this information with our affiliates.
We also automatically receive and record information on our internet
webserver logs from your internet browser, including your internet
protocol address, cookie information and the page you requested.
Information Use
We use information collected on our website:
- To fulfill your requests for services or information,
-
To contact you about our services and those of our affiliates,
based on the preferences you have indicated,
-
To customize the advertising and content you see on our inter-
net webpages,
- To improve our website,
- To develop new services or ideas, and
- To better administer and troubleshoot our systems.
We will give you the opportunity to "opt out" of receiving any unsolicited
information from us or to limit the unsolicited information you receive
from us to information regarding the service for which you requested
information.
Once you register with us and sign in on the CMFHP website, you are not
anonymous to us.
Information Sharing and Disclosure
We may share your personally identifiable information with companies
and organizations that have a product or service that we believe may
be of interest to you. We will, however, offer you the opportunity to
"opt out" of having your personally identifiable information shared
with third parties that are not affiliated with CMFHP. We will also offer you
the opportunity to "opt out" of having your personally identifiable infor-
mation shared with our affiliates.
Even if you have opted out of having your personally identifiable infor-
mation shared with any other person or company, we will send
personally identifiable information about you to other companies or
people when:
-
We need to share your information to provide the service you
have requested,
-
We need to send the information to companies who work on
behalf of CMFHP to provide a service to you (unless we tell you
differently, these companies do not have any right to use the
personally identifiable information we provide to them beyond
what is necessary to assist us),
-
We need to share your information with licensing, accreditation
or regulatory agencies or entities as may be required of us,
-
We find that your actions on our website violate the Terms Of
Use, any of our usage guidelines for specific services and/or
any agreement, or
-
We respond to or initiate subpoenas, court orders or legal
process.
Internet Protocol Addresses
An Internet Protocol (or "IP") address is a number that is automatically
assigned to your computer whenever you request an e-mail or a web-
page from another computer on the internet. Internet web servers
automatically identify your computer by its IP address. When you
request webpages from our website, our internet servers log the IP
address for your computer for the purpose of system administration.
We do not link IP addresses to anything personally identifiable.
Cookies
We will use "cookies" only to recognize you and your access privileges
on our website. A "cookie" is a piece of information that is sent to your
internet web browser from a website and stored on your computer's
hard drive. Each time you return to our site, we are able to identify you
as a previous or registered user. Most internet web browsers are initally
set to accept "cookies". If you prefer, you may set your internet web
browser to refuse "cookies" or to alert you when they are being sent.
It is possible, however, that some parts of our website will not function
properly if you do so.
Ability to Review and Edit Your Information
You may review and approve the information collected about you upon
registration in our database. Upon request, we will remove that infor-
mation from our database or change or correct personal information
that you state is erroneous. You should understand, however, that
information about you in our database might come from a number of
sources.
Security
We will maintain appropriate safeguards to ensure the security, privacy
and integrity of your personally identifiable information.
Changes to this Privacy Policy
We reserve the right to change, modify or otherwise update this policy
at any time. These changes or updates will be effective immediately
upon notice thereof, which may be given by posting on this website, by
electronic or conventional mail or by any other means by which you
obtain notice of changes or updates.
Policies of Other Websites
CMFHP is not responsible for the privacy policies of websites to which this
website may link. Further, due to the diverse nature of our business
our affiliates may have privacy policies that differ from this policy. In
such cases, the privacy policy of the affiliate will be posted and will
supersede the terms of this document.
